The star was established on November 1, 1919, on the first anniversary of the beginning of the battle for the city. It was awarded to people who defended Przemyśl and the Przemyśl region from the Ukrainians. The rank with a plain ribbon was awarded to soldiers and civilians who took part in the defense after November 11 and until the offensive of General Iwaszkiewicz's troops.
On the reverse the artist's signature EM Unger Lwów and the number 417.
